What Every Woman Needs To Know About Oestrogen, Progesterone & HRT

For countless women, the word “menopause” is associated with the phrase “low oestrogen”. This is the narrative many have been told, the explanation offered when hot flashes strike, sleep becomes elusive, or moods swing unpredictably. Conventional medicine largely operates on this premise, with hormone replacement therapy (HRT) frequently prescribed to replenish what is believed to be lost oestrogen.

But what if this long-accepted understanding of menopause isn’t the complete picture? Research is increasingly questioning the singular focus on oestrogen deficiency, suggesting instead a more nuanced reality. Studies indicate that oestrogen levels in menopausal women aren’t plummeting across the board. In fact, levels are often surprisingly high in certain tissues, even as circulating blood levels suggest otherwise.

This challenges the assumption that simply replacing “lost” oestrogen through HRT is an appropriate solution. Indeed, boosting oestrogen further, when it’s already abundant in tissues, creates imbalances along with a plethora of health problems as laid out below. It’s important to understand what’s really happening with hormones during menopause, and why a more informed perspective could change how you approach your own health journey.

HRT Hormone Replacement Therapy

In this 2-hour webinar I will explain why it’s so important for women of all ages to get their oestrogen & progesterone in the optimum ranges especially women who have had a hysterectomy, peri menopausal, menopausal women, women on the birth control pill or those with the Mirena coil fitted. You may ask the question ~ WHY?

Sadly, you will find that most women have been blindly prescribed HRT without any testing or monitoring on how these hormones are functioning, what levels are being achieved or how these hormones are being metabolised when on HRT.

For example, in one woman HRT could be having significant benefits and, in another woman, could be creating the perfect environment for cancer & obesity.  How? If, you have high levels of oxidative stress, poor methylation status, low glutathione levels or a genetic weakness to methylate then it is far more likely oestrogen is going to go down the more deleterious pathways as explained later like the 4-OH Hydroxy pathway & 16-OH Hydroxy pathway creating a more cancer prone environment.  All these markers are easily tested via urine, blood, stool & DNA testing please click on my lab testing page for more details.

Many women are already oestrogen dominant even before they are put on HRT and then through the addition of HRT, they become even more oestrogen dominant making them a lot more vulnerable to a plethora of health issues as listed in the diagram below.

Symptoms of Estrogen Dominance

For example, lets look at oestrogen did you know there are 3 oestrogens oestrone (E1), oestradiol (E2) and Oestriol (E3).  Oestrogen is detoxified down three pathways which primarily takes place in the liver. There is a healthy pathway known as the 2-OH Hydroxy Pathway and we want to increase the detoxification of oestrogen down this pathway.

Another pathway is the 4-OH Hydroxy pathway, this is an unstable pathway that increases the risk of cancer. We want these levels to be low for women especially if there is a family history of breast or ovarian cancer.

The third pathway is the 16-OH Hydroxy pathway. It is involved in the detoxification of estriol which is a weaker form of oestrogen. This pathway can also be associated with proliferation so can make things grow, so can be worse for endometriosis and cancer.

There are also three phases of healthy detoxification Phase 1 & 2 primarily which takes place in the liver and phase 3 in the bowels so it’s essential to have good liver & bowel health if you want to eliminate hormones especially oestrogen otherwise it will get reabsorbed back into the body creating more of an oestrogen dominant environment.

The DUTCH hormone urine test shows the levels of oestrogen, progesterone, and testosterone that a person has, and this is important information as it can show whether the hormone levels are optimal or not.  Only the DUTCH test can show how your body is metabolizing these hormones.

WHY is this important? If you have high oestrogen levels which is quite common for many women as mentioned above, the DUTCH hormone test can help to show why the hormones are high. You will be able to know if it is an issue with making too much oestrogen or is it an issue with phase one of liver detoxification, or a problem with the methylation of oestrogen by the COMT gene.

There are many nutrition, supplementation, lifestyle & detoxification interventions that can be done to help optimise oestrogen’s being pushed down the healthy pathway and minimised from going down potentially carcinogenic pathways which I will discuss in this webinar. 

But how would you know how your oestrogen is being metabolised if you don’t get it tested and blood oestrogen testing for peri & menopausal women is almost completely useless plus blood tests don’t tell you how your oestrogen is being metabolised. This information is only acquired with the DUTCH (Urine Test) hormone test and not assessed in blood or saliva tests.

The GI MAP stool test can tell us whether stage 3 of detoxification is functioning optimally by measuring the enzyme beta glucuronidase if this is high it means you are reabsorbing oestrogen back into the body along with toxins which also should have been eliminated via the stool.

Phase Three Detoxification

What about progesterone this is usually very low in peri & menopausal women or women who have had a surgical menopause creating an oestrogen dominant environment which can create significant health problems as the diagram below clearly shows.

Oestrogen is essential for women’s health no doubt about that but when there is not enough progesterone around to counter the effects of oestrogen then oestrogen will undoubtedly, over a period of time, cause significant health problems. So, it’s essential to know your progesterone levels along with your progesterone to oestrogen ratio which should be much more in favour of progesterone to oestrogen though I very see this from the lab testing that I have run on women over several years.

Not only are there 3 different oestrogens but there are also 2 different oestrogen receptors ER-a & ER-b. Binding of oestradiol to the ER-a receptor leads to breast/endometrial cell proliferation, while binding of oestradiol to ER-b blocks proliferation.  In addition, Progesterone decreases the binding affinity of oestradiol for ER-a while increasing its binding affinity for ER-b.  ER-a is an accelerator, while ER-b and Progesterone serve as brakes.  This is another reason why it’s essential to have optimal levels of progesterone to oestrogen.

Role of Estrogen and Progesterone

Your nutrient status and toxic load will also significantly affect how your hormones work and most people today have multiple nutrient deficiencies or excess along with a high toxic load which can prevent or overstimulate hormones from functioning correctly.
