It is important that the Diary reflect a typical week for you, so that I can analyse your current eating and drinking habits and advise appropriately.

I recommend that you use a notebook, journal, mobile phone or other convenient method to keep track of what you are eating and drinking for a week, and then submit the below form once you have 7 days worth of data.
